The MISEV guidelines have become essential to extracellular vesicle research, yet their purpose extends far beyond prescriptive rules or rigid checklists. Five fundamental questions form the foundation of these recommendations: What terminology do you use and what does it mean? Where did your EVs originate? How were they isolated, concentrated, characterized, and stored? Can you confidently attribute findings to EVs versus other components? Have you documented methods thoroughly enough for others to replicate your work?

These questions reveal MISEV's true intent—not to constrain innovation, but to strengthen scientific rigor across the entire field. Isolation methods significantly impact EV composition and downstream analyses, meaning two groups studying identical EV types with different techniques may reach contradictory conclusions. Understanding how to apply MISEV principles effectively transforms them from perceived obstacles into powerful tools that advance both individual studies and the collective progress of extracellular vesicle science.
